Options for treating fibroids

[vc_row][vc_column][vc_column_text]When it comes to treatment options for fibroids, there are quite a few but it is important to choose a treatment modality based on your goals and required outcomes. And again, when the fibroids aren’t impeding any plans to get pregnant, treatment may not be required. Many research studies have proven that asymptomatic fibroids do increase maternal and fetal morbidity in pregnancy and as for the cumulative risk of adverse obstetric outcomes in pregnant women having a fibroid diagnosis is concerned; around 48.8% were found to result in a C Section and at least 16.1% had a preterm labor.

Another factor to consider when it comes to whether or not fibroids need treatment is the location of the fibroids. Certain fibroids like the Subserosal myomas will not impair fertility as they don’t indent the uterine cavity. Women with Subserosal fibroids had no differences in their fertility outcomes as such patients also were found to have IVF-ICSI outcomes that were comparable to those without leiomyomas. Some studies show that women with intramural fibroids that don’t distort the uterine cavity appear to have decreased fertility and increased pregnancy loss when compared with women without such tumors.

Let’s look at the various non invasive treatment options for fibroids if pregnancy is likely to be planned in the future.

A Laparoscopic myomectomy can help remove the fibroids robotically. The procedure is less invasive with better recovery timelines as well. A myomectomy may need to be done before being scheduled for assisted reproduction cycles.

A magnetic resonance imaging-guided focused ultrasound surgery (MRgFU) uses ultrasound waves to destroy fibroids. The waves are directed at the fibroids through the skin with the help of magnetic resonance imaging. This technique required less analgesics, short hospital stay and has faster recovery period as well. Some studies have found that the incidence of follow-up treatment with MRgFU is comparable to other uterine sparing techniques and also lower rates of recurrence are seen in older women above the age of 46. Successful term pregnancies following MRgFU have also been noted in few cases.

In the minimally invasive Laparoscopic Radiofrequency Ablation, controlled heat is generated to change the fibroids from hard to soft, reducing the painful symptoms associated with uterine fibroids. The process is repeated until every fibroid is treated. This treatment does not affect any healthy surrounding tissue in the uterus. Based on the size of the fibroids, the symptoms will begin to ease in 3 to 12 weeks’ time.

The Uterine artery embolization destroys fibroid tissue and provides an alternative to surgery to remove fibroids (myomectomy). This technique is especially recommended in premenopausal over 40 years, yet women of reproductive age. Findings indicate many benefits for women desiring fertility who undergo UAE over traditional myomectomy. This treatment option has been shown to be successful for women with symptomatic myomata. Long-term results have revealed a 90% cure rate of myoma symptoms. It is imperative that proper fertility counseling and assessment is made prior to UAE to ensure that the

patients’ fertility goals have the best chance of being achieved.

In Radiofrequency ablation (RFA) thermal energy-based systems are made use of to treat uterine fibroids that are symptomatic. The thermal ablation devices involve hyperthermia which is heating of tissue resulting in reductions in fibroid volume, associated symptoms and the need for reintervention.
